The exceptions that matter
California: Oak Longevity — the cheapest verified semaglutide at $133 — doesn't serve CA at all, and both bmiMD and MEDVi bill their all-inclusive semaglutide at $379.99 in California and North Carolina versus $289/$299 elsewhere (state pharmacy-sourcing costs pass through). North Carolina: same two surcharges apply. Most other programs publish 45–50-state footprints; NexLife publishes 50-state availability. Program pages carry each footprint as published, with dates.
Why states differ at all
Three mechanisms: state boards of pharmacy license which compounders may ship in (the 503A layer), a few states regulate compounded GLP-1s or telehealth prescribing more aggressively, and cross-state pharmacy sourcing costs get passed through as surcharges. Brand channels (LillyDirect, NovoCare, retail KwikPens) are nationally uniform — one more quiet advantage of the brand route.
