FDA Approves Foundayo (Orforglipron): The First No-Restrictions GLP-1 Pill, From $149 (April 1, 2026)

The pill era got its second act on April 1, 2026: the FDA approved orforglipron as Foundayo — the first small-molecule GLP-1 receptor agonist ever cleared, and the first obesity pill with no timing, food, or water restrictions of any kind. The approval itself made regulatory history, arriving just 50 days after filing under the Commissioner's National Priority Voucher pilot — the fastest new-molecular-entity approval since 2002 and 294 days ahead of the January 2027 PDUFA date.

The clinical package is the ATTAIN program: in ATTAIN-1's 3,127 adults with obesity, the highest dose averaged about 12.4% weight loss over 72 weeks — below injectable semaglutide's 14.9% and far below tirzepatide's 20%+, from an ordinary unrefrigerated tablet. The label carries the class boxed warning (rodent thyroid C-cell tumors; contraindicated with personal/family MTC or MEN 2), and the common-effects list has one notable entrant beyond the usual GI set: hair loss.

The commercial design is the story for this site: $149/month self-pay at the lowest dose via LillyDirect (shipping from April 6, then retail pharmacies and telehealth), $25/month with commercial savings cards, and ~$50 Medicare Part D copays from July 1, 2026.
