GIP was long dismissed as a weight-loss target — it is secreted from K-cells and was historically associated with fat storage. The modern picture is different: in combination with GLP-1 agonism, GIP receptor activity appears to add central appetite effects through distinct hypothalamic populations, improve adipose tissue insulin sensitivity and lipid buffering, and — notably — attenuate the nausea signaling that limits GLP-1 dosing. That last point matters clinically: it is one proposed reason tirzepatide reaches high, effective exposures with GI tolerability comparable to semaglutide despite greater weight loss.

The pharmacokinetics that make it weekly

The fatty-diacid chain binds albumin, protecting the peptide from degradation and giving tirzepatide a half-life of about five days — hence once-weekly injection and the ability to shift a dose day by a couple of days without losing steady-state coverage. Steady state arrives after roughly four weeks at a given dose, which is also why the titration ladder moves in four-week steps.

What the mechanism predicts — and the trials confirmed

Dual agonism predicts larger weight loss than GLP-1 alone, and the trials delivered: 15.0% to 20.9% average reductions across doses in SURMOUNT-1 versus placebo's 3.1%, and 20.2% versus semaglutide's 13.7% in the head-to-head SURMOUNT-5. It also predicts strong glycemic effects — tirzepatide produced the largest A1C reductions ever seen in the SURPASS diabetes program — and metabolic improvements in lipids, blood pressure, and liver fat that track the weight loss.
